Purpose:
The ultimate goal of every delivery is a healthy mother and a healthy infant. Therefore, if a mother’s or her infant’s health is at risk, the provider will likely use an intervention and work towards a safe delivery for the mother and baby. We would describe this as a medically necessary intervention. All medical interventions have risks and benefits.
Research provides evidence that there are hospital practices that support breastfeeding thereby increasing the mother’s change of breastfeeding successfully. However, there are also hospital practices that hinder breastfeeding thereby making it significantly more challenging for the mother to be successful at breastfeeding.
The purpose of this assignment is to provide you with the opportunity to gain the knowledge needed to educate families in the community on how to make informed decisions regarding hospital practices used during labor, delivery, and the postpartum period. Specifically, this assignment focuses on enabling you to gain an understanding regarding: 1) which hospital practices support breastfeeding and which practices hinder breastfeeding, 2) what are the medical reasons for interventions utilized during labor, delivery, and the postpartum periods; and 3) research on how each practice can influence breastfeeding.
This assignment has three parts A-C and instructions for completing each part are described below:

During the hospital stay, provide mothers with practical support to enable them to initiate and establish breastfeeding and manage common breastfeeding difficulties.
During the hospital stay, mothers should be coached on how to express breastmilk as a means of maintain lactation in the event of their being separated temporarily from their infants.
During the hospital stay, maternity facilities should enable mothers and their infants to remain together throughout the day and night (i.e., rooming in)

Rationale for using Pitocin:
Pitocin is a synthetic or human-made version of oxytocin used to induce labor when it is medically indicated.
How does Pitocin hinder breastfeeding breastfeeding?
Pitocin can result in edema or swelling in the extremities, including the breast and nipple. Edema results in swelling Edema in the breast and nipple tissue results in flat nipples, which make it difficult for the baby to latch on. Edema after birth can take as long as two weeks to resolve. Edema associated with Pitocin is worse when the mother receives many liters of IV fluid and has an epidural. Under these circumstances, a woman may also experience delays in lactogenesis. Lactogenesis II is when the body stops producing colostrum and starts producing mature milk (Nommsen-River et al., 2010). Delays in lactogenesis II negatively impact breastfeeding as less milk is available for the baby.
